All it takes is one call to AllCare Orthopaedic Center to schedule an appointment with Dr. Leventhal. An AllCare Front Office Technician will make an appointment and answer your questions about AllCare Orthopaedic Center services. AllCare Orthopaedic Center is open Monday through Friday, 8:00 am to 5:00 pm.
Our goal is to provide an on-time practice. All our patients deserve the courtesy of prompt service. We ask your help by requesting that you arrive at the scheduled time of your appointment. Also, if you are delayed or must cancel, please take a minute to call our office, 217-342.-5800.
What To Expect With Your
First Doctor Visit.
You will be greeted by our receptionist. You will be asked to complete a health history questionnaire either prior to your first visit or upon arrival. This questionnaire is a vital part of your examination and we ask that you take care in completing it completely and accurately. If you need assistance, please feel free to ask any one of our staff members.
In addition to the health history questionnaire, you will also complete a form with your insurance information. As with the health history, please take care to complete this completely and accurately. We are happy to file your insurance for you, but your assistance in providing the correct information is necessary.
Please bring the following items with you on the day of you first visit.
• Any medications you are taking.
• The name and address of your medical doctor.
• Your valid insurance cards (including your Medicare card).
• Copies of any and all tests or films taken.
(i.e. x-rays, bone scans, MRIs, EMGs)
• Previous x-rays and test results relating to your injury/illness.
• Previous medical records relating to your injury/illness.
• Referral letters from your employer/primary care physician,
(i.e. workers compensation statement, HMO/PPO referrals)
• Interpreter if needed.
• Please wear attire that is comfortable during your examination.
When you have completed these forms, you will be taken to an examination room by one of our medical professionals. There they will ask you questions to provide additional information for the physician. Your vital signs will be taken and other tests may be performed based on your chief complaint at the time of the visit and your past medical history.
Being an informed patient is one of the most important contributions you can make to your treatment and your personal well being. You'll find it much easier to be involved in your treatment if you know what is expected of you.
Please do not hesitate to ask questions about the results of your examination, or the treatment recommended by the physician. Your diagnosis and all possible treatments will be completely explained to you.
